We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information. We will not use or share your information other than as described here unless you tell us we can in writing. If you tell us we can, you may change your mind at any time by letting us know in writing.
How we may use and disclose your health information
- Treatment. We use and share your information with dentists, hygienists, assistants and other providers involved in your care, including specialists and dental laboratories.
- Payment. We use and share your information to bill and get payment from you, your dental insurance or another payer, for example to confirm benefits or submit claims.
- Health care operations. We use and share your information to run our office, improve care and contact you when necessary, for example quality reviews, staff training and business planning.
- Appointment reminders and treatment information. We may contact you by phone, text, email or mail to remind you of appointments or tell you about treatment options and services that may interest you. You can ask us to use a different method or to stop.

Other uses and disclosures allowed or required by law
We may share your information without your written authorization when the law allows or requires it, including: to comply with state or federal law; for public health and safety, such as reporting suspected abuse, neglect or domestic violence, or preventing a serious threat to health or safety; for health oversight activities and audits; in response to a court order, subpoena or lawful request from law enforcement; to a coroner, medical examiner or funeral director; for workers' compensation claims; for specialized government functions such as military or national security; and for organ and tissue donation requests.

Your rights
- Get a copy of your records. You can ask to see or get an electronic or paper copy of your dental and billing records. We will provide a copy or summary, usually within 30 days, and may charge a reasonable, cost-based fee.
- Ask us to correct your records. You can ask us to correct information you think is incorrect or incomplete. We may say no, but we will tell you why in writing within 60 days.
- Request confidential communications. You can ask us to contact you in a specific way (for example, a home or office phone) or to send mail to a different address. We will say yes to all reasonable requests.
- Ask us to limit what we use or share. You can ask us not to use or share certain information for treatment, payment or operations. We are not required to agree, except that if you pay for a service in full out of pocket, you can ask us not to share that information with your health insurer, and we will say yes unless the law requires us to share it.
- Get a list of those we've shared information with. You can ask for a list (accounting) of the times we've shared your information for six years prior to the date you ask, who we shared it with and why, other than for treatment, payment, operations and certain other disclosures. We provide one list a year for free.
- Get a copy of this notice. You can ask for a paper copy of this notice at any time, even if you agreed to receive it electronically.
